Patches

Finish Patch in the editor does not cause the patch comment to appear in
print-system-modifications.  Even calling LOAD-PATCHES will not cause the
patch version to show up in the modifications.  I propose the following:

When a patch is started with ADD PATCH, a NIL comment should appear in the
system modifications just like it does in the patch file.  Then when the
patch is finished it should replace the NIL in the comment field with the
actual comment.  This has the wonderful advantage that anyone can tell that
there is a patch in progress as well as the result.
