Issue IGNORE-VARIABLE, v1

In the discussion leading up to this issue being proposed, and in the proposal
itself, there have been a number of comments about it being an error to have a
duplicated name in a binding list.  I'm not trying to claim that doing so is a
good idea, but I've never been able to find anything in CLtL that says doing so
is wrong.  Also, I believe that the results are actually well defined for
sequential binding forms (though not for parallel binding forms).  If CLtL does
say something about this, would somebody please point it out to me?
