Issue STREAM-ACCESS

Proposal STREAM-ACCESS:ADD-TYPES-ACCESSORS, passed at the Hawaii meeting, says
that the types TWO-WAY-STREAM and ECHO-STREAM are mutually exclusive.  This
seems inappropriate -- the two seem to be equivelent except that echo-streams
provide the additional echoing functionality.

We wish to add an ammendment to require (or allow, if somebody claims problems
with this) ECHO-STREAM to be a subtype of TWO-WAY-STREAM.
