In System 78.44, ZMail 38.5, microcode 843, 60Hz, on Lisp Machine One:

In trying to respond to a qsend, when the response was sent it bombed with
with a window system problems using the cold load stream error.  The error
was a failed disk read on unit 0, cyl 56, surf 12, sec 2 which also lost
on several retry attempts.  The lossage occurred while in SI:DISK-RUN called
(via other functions) by SYS:PAGE-IN-WORDS.
