issue TEST-NOT-IF-NOT

I would vote for the FLUSH-TEST-NOT option if the COMPLEMENT function
were added to the language in its place.  I think that FLUSH-ALL would
have been a terrific idea 5 years ago but doing it now would cause too
many problems for existing code.  I expect that even if FLUSH-TEST-NOT
passes, many implementations would continue to provide the :TEST-NOT
arguments anyway to support existing applications.
